OPI — New social security reforms designed to control today's inflated Medicare costs are scheduled to go into effect nationwide, beginning Oct. 1. Just how these reforms will affect the health care of the nation's Medicare patients is unknown, but one thing is certain: With the new methods comes an array of new terminology that already is baffling both patients and health care providers. Terms, for example, like DRGs (diagnosis-related groups), case-mix reimbursement and prospective payment. To help nursing and allied health professionals understand these new terms and their implications, an expert on the subject, Paul L. Grimaldi, Ph.D., will conduct an all-day workshop Friday Oct. 21, at the Mountain Area Health Education Center in Asheville. Grimaldi, whose seminar will focus on Medicare's new system of reimbursing hospitals and controlling hospital costs, is director of National Health Care Practice at Coopers and Lybrand, a New York-based international certified public accountant firm. Formerly, he was director of health economics research services for the New Jersey Hospital Association. Grimaldi also is known for his book, "DRGs: A Practitioner's Guide," which he wrote with co-author Julie Micheletti, B.S.N. The book has been described by Michael Doody, president of the National Health Care Financial Management Association, as "... the most important (book) as you begin to prepare you institution for DRG's" The term diagnosis-related groups (DRG) refers to groups of patients diagnosed with the same or related medical problems. Under the new Medicare reforms, the government will reimburse hospitals predetermined amounts based on the average cost of treating patients in the same DRG. This new method, called prospective payment, differs considerably from the current system whereby Medicare reimburses hospitals for individual patient costs. Grimaldi's seminar will be held from 8:30 a.m. until 4.30 p.m. in MAHEC's lecture hall. It is sponsored by the continuing education department of Western Carolina University's School of Nursing and Health Sciences, Region I of the N.C. Medical Record Association and MAHEC. Preregistration by Wednesday, Oct. 12, is recommended. Cost of the seminar is $45, including lunch, refreshments and materials. OPI—Therapeutic Recreation, two words many individuals cannot even spell, is swiftly becoming important for providing treatment for people with special needs. Western Carolina has recently introduced Therapeutic Recreation to its curriculum. But according to Dr. Edward Kesgen, the man who energetically accepts his chore of establishing Western's program. Therapeutic Recreation has been around and practiced for centuries. Positive use of leisure activity has been recognized as being therapeutic since ancient times. Early civilizations such as the Egyptians, Greeks, and Romans established spas for treatment of what Dr. Kesgen calls, various "Melancholias." Therapeutic Recreation emerged as a profession after World War II, when it became apparent that medical treatment was not sufficient for the rehabilitative needs of soldiers who wer suffering various combat disorders. Dr. Kesgen further explains his definition of Therapeutic Recreation by stating that, "It is the utilization of psychological, emotional, and/or social behaviors to assist individuals in establishing and expressing and independent lifestyle." Dr. Kesgen, who will be teaching many of the courses in the curriculum, has brought an impressive academic record to Western Carolina. After obtaining a double major in Psychology and Philosophy from Seton Hall, he moved on to Springfield College (home of the Basketball Hall of Fame) and received a graduate degree in Community and Outdoor Recreation. Dr. Kesgen's final stop was at Texas A&M, home ofthe fighting Aggies, where he received the Ph.D. degree in environmental interpretation. He has taugiit at several universities including Texas A&M, and even found time to do some military service. When asked why he chose WCU and what attracted him most, Dr. Kesgen responded, "Western has a reputation of being a growth school; there is an exciting future ahead." WCU's growth characteristic explains the decision for transforming the recreation leadership program into the more needed therapeutic recreation course of study. This change was made to "keep up with the demands of the times." Another of WCU's attractions is its abundant natural resources and beautiful surroundings, which Dr. Kesgen hopes to incorporate into WCU's program. An important note about WCU's therapeutic recreation program that distinguishes it from those of other schools in North Carolina is that WCU is the only school in the state that offers a B.S. in Therapeutic Recreation. Other schools in the state only offer therapeutic recreation options in a general recreational and parks course of study. Expanding employment opportunities make this particular degree even more attractive. Jobs are available in hospitals, nursing homes, residential treatment centers, specialized summer camps and outdoor adventure programs, and large municipal recreation and park departments.
