  • IMPORTANT—READ THIS CARD CAREFULLY You are eligible to receive Class "B" or "B—1" family allowance checks only so long as you are dependent for support on the person in service. In order to keep the Navy Department informed of your circumstances in-regard to dependency it is necessary that you complete the other card in this envelope and mail it at once. It is already addressed and requires no postage. Before mailing the card, check the square before the one (and only one) statement which explains your case. Read the other card carefully, and then check the statement, that applies to you. If you check Statement 3, further information will be requested from you at a later date. Then sign the card and fill in your present address and the full name and service number of the person in service. The card must be returned promptly — Mail it at BUREAU OF NAVAL PERSONNEL
